    Just to close this out. I decided to check the wire connections at the IAC and found one of the wires in the connector had worked loose. I reinserted the wire and all seems to be working as it should.

    That was a good thing to check. Better to find a wire loose than to buy a new IAC and not solve the problem. Your symptoms sounded like a bad IAC, but I hadn't thought of the possibility of a good IAC with a bad wiring connection. Guess it always pays to walk the entire fault tree branch by branch. You just never know which branch the fault lies on.
